PREPARING FOR MY APPOINTMENT

Please arrive to the office 15 minutes prior to your first scheduled appointment to fill out the required paperwork. You may also call our Clinical Coordinator to fax or email you the required forms.

Bring with you:

  • Drivers license or form of I.D.

  • Current insurance card and insurance referral forms (if applicable)

  • Medical records, films, lab results from current treatment and/or your primary care physician

    • If you request medical records be sent to our office, please confirm that we have received them before your appointment

  • Current list of all medications you are taking

  • Medical history

  • List of questions and concerns to address during your appointment

BILLING AND INSURANCE

If you are insured, we will bill those insurance plans with which we have an agreement. However, it is ultimately your responsibility to become familiar with the details of your insurance plan coverage. We recommend you contact your insurance company prior to any service so you may understand your allowable benefits.

If you have a PPO or HMO insurance plan, we will collect the required co-payment, co-insurance, and any deductible that is due at the time of the visit. In the event that your health plan determines a service to be “non-covered,” we will bill you, and payment is due upon receipt of that statement. Any amount not paid by your insurance company within 30 days will be billed to you.

If your insurance coverage is with a plan that we do not have an agreement, payment is expected in full at the time of service. As a courtesy, we will submit a claim to your insurance company on your behalf.

Your physician may or may not be a participating Medicare provider. If your physician is a participating provider, we will bill your Medicare insurance and your supplemental, if you have one.

There is a public parking structure on Broxton Ave (behind the Whole Foods) which provides 2-hours free parking for patients. Metered parking is also widely available on Gayley Ave.
